Anterior versus Lateral Total Ankle Replacements : A Different Reoperation Profile
Description
In this overview presented by Oliver Gagne, we explore recent advancements in ankle arthroplasty over the past decade, focusing on two primary approaches: anterior and lateral total ankle replacement. Oliver introduces significant innovations including trabecular metal press-fit implants and cross-linked polyethylene, emphasizing the importance of a staged approach for optimal candidate selection prior to surgery.
The discussion highlights the distinctive surgical techniques associated with each approach, where the lateral approach necessitates fibular osteotomy and allows for burred circular cuts, whereas the anterior approach facilitates flat cuts and offers direct visualization of the anatomical structures.
Oliver presents data from a retrospective study comparing reoperation rates between both techniques, stating that the anterior group had 7 reoperations affecting 5 out of 67 patients, while the lateral group faced a much higher rate, with 33 reoperations among 22 out of 48 patients. The presentation acknowledges the skew in case types which introduces bias into the findings, and concludes with implications for future practices regarding hardware choices to potentially reduce reoperation rates.
Citing a study published in FAI, Oliver invites further reading for those interested in deepening their understanding of this evolving field.
