BOFAS Interactive Instructional Case 2 Introduction: 40 Year Old Persistent Pain from Osteo-Chondral Lesion Talus after First Microfracture

Description

In this recording of a clinical discussion, Howard Davies introduces a case involving a 34-year-old policeman and elite runner suffering from persistent deep ankle pain due to an osteochondral lesion, first identified two years prior. The lesion, located in the centro-lateral portion of the talar dome (zone 9), has previously been treated with microfracture techniques. The patient remains proactive in his recovery, adhering to physiotherapy despite carrying symptoms, including anterolateral joint line tenderness and a slight ankle effusion. An MRI reveals the lesion measures approximately 10 by 10 millimeters.

Following the case presentation, Davies introduces three experts from the British Orthopaedic Foot and Ankle Society (BOFAS) who will share their treatment methodologies. Maneesh Bhatia from Leicester advocates for repeat arthroscopy in cases involving microfracture, while Nick Cullen from Stambaugh supports the use of autologous matrix induced chondrogenesis. Finally, Anand Pillai from Manchester presents his argument for osteochondral grafting as a treatment option. The discussion is set to provide insights on various advanced therapeutic strategies for managing this condition.
