Eclipse Stemless Shoulder Replacement

Description

This video provides a detailed overview of performing a shoulder hemiarthroplasty using the Arthrex Eclipse stemless system. The procedure begins with positioning the patient in the beach chair position and making a deltopectoral incision to access the shoulder joint. Careful dissection is performed to identify and mobilize important structures, including the cephalic vein and the conjoint tendon, leading to the subscapularis and biceps tendon.



Critical steps include reflecting the subscapularis from the lesser tuberosity and exposing the humeral head, which is found to be degenerated due to avascular necrosis. Osteophytes and loose fragments are removed, and a cutting jig is aligned to guide the humeral cut. Pins are inserted for stabilization, and an oscillating saw is used to excise the humeral head.



After determining the correct size of the trial humeral implant, a pilot groove is created for the centralizing peg, which is used to guide the insertion of the definitive implants, including the trunnion and the cage screw, with specific care not to engage on the far cortex. The humeral head is then placed onto the trunnion and secured, followed by the repair of the subscapularis using sutures.



Finally, the skin is closed with a subcuticular suture, and the stability of the repair is checked through arm rotation.
