Lelli's Test for ACL
Description
The video tutorial presents a comprehensive demonstration of Lelli's test, also known as the lever test, which helps assess the integrity of the anterior cruciate ligament (ACL) in a patient's knee. The presenter begins by explaining the significance of the test, especially in acute situations where other tests, like the Lachman test, may be challenging to perform due to pain. Lelli's test can be conducted shortly after a knee injury, providing valuable information regarding the functionality of the ACL.
The presenter outlines the procedure, first ensuring that the patient is supine and then placing a fist under the calf. By applying moderate force on the heel, the demonstrator visually illustrates the test's outcome based on the heel's response to the pressure, indicating whether the ACL is intact or damaged. A positive test shows the heel lifting off the examination couch, whereas a negative result means the heel remains on the couch, clarifying significant ACL dysfunction.
Two patient examinations are provided as examples: one with a negative test result and another with a positive result, the latter indicating a potential complete tear of the ACL. The presenter emphasizes that Lelli's test is underutilized in clinical practice, encouraging its adoption among surgeons due to its effectiveness and ease of use in assessing ACL integrity.
