Distal Radius Fracture Manipulation Quality Improvement Project Utilising an Orthopaedic-Led Outpatient Pathway and Bier Block Regional Anaesthesia

The presentation delivered by Hasan Daoud, a new CT1 doctor in Wakefield, discusses a quality improvement project focused on the manipulation of distal radius fractures. He explains the commonality of this type of injury and the variability in management depending on hospital guidelines. Traditional practices often involve emergency department (A&E) staff performing manipulations, which includes techniques such as hematoma block. By assessing data from 234 wrist fractures over a three-month period, they found significantly lower rates of surgery and reduced A&E wait times when orthopedic specialists performed manipulations. Subsequently, an orthopedic-led outpatient pathway was developed in Derby utilizing regional anesthesia (Biers block) to create specific time slots for fracture management, enhancing patient care and satisfaction. Data collected post-implementation indicated improved outcomes, including fewer surgeries required and high patient satisfaction, demonstrating the effectiveness of the new protocol in managing distal radius fractures.
