Open Reduction Capsulorrhaphy & Acetabuloplasty (ORCA): A One Stop Surgical Treatment for Infant DDH

In this presentation, Amanda Rhodes, a Specialist Trainee in the Wessex Deanery, discusses a surgical technique known as ORCA (Open Reduction, Capsulorrhaphy, minimal Acetabuloplasty), developed at Southampton Children's Hospital for treating developmental dysplasia of the hip (DDH) in infants. The talk begins with an overview of current treatment options, highlighting the goals of surgery to stabilize the hip joint and promote healthy growth of the acetabulum and femoral head. Amanda emphasizes that traditional surgical options like pelvic osteotomy can be extensive and invasive.



She explains Southampton’s method that differs from classical approaches by focusing on stimulating acetabular growth rather than immediate reorientation. Providing evidence from a twelve-year follow-up study including 100 cases, she shares their findings which show positive normalization of the acetabular index over time, with a low re-dislocation rate and a minimal need for additional surgeries.



In conclusion, Amanda advocates for the ORCA technique as a less invasive yet effective procedure that complements open reduction, supporting its routine use alongside this primary surgery for infant DDH treatment.
